The Centers for Medicare and Medicaid Services recently announced that it will stop reimbursing hospitals for treating eight reasonably preventable conditions - including pressure ulcers - in October 2008. That simple announcement has put hospitals across America into a state of anxiety and with good reason. Up until now, the majority of hospitals have been focusing on pressure ulcer treatment, rather than prevention.
It's understandable, since the number of hospital patients who develop pressure sores has risen by 63% over the last 10 years and nearly 60,000 deaths occur annually from hospital-acquired pressure sores. That explain why hospitals tend to focus on treating the ulcers and the extreme pain and life-threatening infections that follow, rather than preventing them.
But that's not the only toll. According to the Agency for Healthcare Research and Quality (AHRQ), the average stay for patients admitted to the hospital for treatment of hospital-acquired pressure sores was 13 days, with an average cost of $37,500 dollars per hospital stay. Unless hospitals can afford to continue simply treating pressure sores as they occur without Medicare to subsidize their costs, something must be done.
Pressure sores can initially develop when the blood supply to a patient's skin is cut off for more than two or three hours, or is aggravated by constant pressure on the skin and tissues. When a patient is immobile or has difficulty moving after surgery or procedures, even the slightest friction burns created by bed sheets, wheelchairs or other surfaces can quickly turn into pressure ulcers.
One problem, is that pressure ulcers can initially be hard to identify, sometimes looking like nothing more than a pink or red spot on hard to see areas like skin folds or bony protrusions. And when they do occur, complications such as bone, blood, and skin infections, can quickly develop. Not only do patients suffer; but so do hospitals - treating pressure sores often costs more financially and in personnel hours than the measures that could have prevented them. That doesn't even take into account the cost of litigation hospitals and care facilities can face, for putting their patients at risk when pressure ulcers develop.
So what can we do? We need to put proactive medicine to work by transforming our basic pressure sore protocols into pressure ulcer prevention programs. I know what you're going to say. Your facility already has a program in place to identify patients who are at risk to develop pressure ulcers. Maybe you even have a wound management professional to treat patients and instruct your staff on proper procedures. But as hospitals across the country have seen, if pressure ulcers are still occurring, following basic protocols may not be the answer.
For example, Thibodaux Regional Medical Center in Louisiana already had a better than average pressure ulcer rate. But when their quality improvement data detected an increase in the ulcer rate in late 2003, they decided to launch a Six Sigma project to address the issues. After examining the data, they were surprised at the outcome. First they determined that the tools they had been using to analyze their care were unreliable. Second, some preventative measures they relied upon, like using a specific type of patient bed, wasn't having as much of a positive effect of patient outcomes as small improvements had - for example, the way smaller tasks were performed on the nursing floor. The recommended changes that came out of the project included simple shifts in procedure including reorganization of wound care products on nursing units and creating task lists for CNAs.
Additional solutions included:
Posting a turning schedule in patient rooms to identify need and document results of Q2H turning of patient.
Unit educators to address skin issues during annual competency testing
Reporting on patient skin issues during shift changes
The result? A sixty percent reduction in the overall nosocomial pressure ulcer rate, with an annual cost avoidance of about $300,000.
The best way to create and implement a new pressure ulcer prevention program is to model the successful programs instituted by other hospitals, and recreate that success in your own facility. The Ultimate Marathon Training Schedule
It's really hard to determine a good marathon training schedule, let alone sticking with it. There are too many temptations to eat an entire chocolate cake in one sitting (I know.)
Since I am now the voice of experience, I can give you some tips on what you SHOULDN'T do so that you'll successfully stick with your schedule. It's very important to remember a few things after you've determined your marathon training schedule.
- Running very quickly for three minutes does not substitute jogging for thirty minutes. You will actually need to do the jogging that you've outlined on your schedule.
- Get creative and involve your family when you can't find the motivation to run every weekend for three months. Here's what I did. My husband dangled a snickers bar out the window of the car while he drove three feet in front of me.
- Breaking in your jogging shoes before the marathon does not include a meat mallet whatsoever. It actually means that you have to run with the shoes on.
- Don't choose Jack Daniels or Captain Morgan as a 'running partner'-- get a real human.
- No, you do not tape your marathon training schedule to your read end in order to stick with it.
- Resist the temptation to hitchhike when you're jogging those last three miles back home.
- Taking a little nap in a ditch by the road is not only inadvisable, it's downright unsanitary.

Mediterranean Diet: Oranges, a Major Player in the Prevention of Heart Disease
Oranges have been from time immemorial a staple food in the Mediterranean countries. They have also been a major player in protecting the people of that region from heart disease. Thus, if you never thought of oranges as a “must” food for your heart, here are some good reasons to start doing so:
? Oranges and vitamin C Oranges are very rich in vitamin C. This vitamin has the following effects in our body: ? ? Calcium. Oranges contain a good amount of calcium, a mineral extremely important in maintaining normal blood pressure; it is required for nerve transmission and regulation of heart muscle contraction. To reap the full benefits of oranges, grate a tablespoon of the peel and use it to flavor tea, salads, yogurt, soups, and cereals.
? Oranges and Fiber One orange contains about 3 grams of fiber: 60 percent is soluble fiber and 40 percent insoluble. Both types are important for our health, but soluble fiber is the one that lowers cholesterol. Pectin, the soluble fiber that's found mainly in the skin around each orange section and in the peel, helps trap and eliminate cholesterol from the body. Try to eat a little of the white part of the orange peel; it contains half of the fruit's pectin supply.
Vitamin C Supplements
Vitamin C supplements do not accomplish in our body what fresh oranges do; make an effort to get your vitamins from fresh foods. However, if you decide to continue taking vitamin C supplements, keep the following in mind:
? Do not exceed 2 grams of vitamin C a day because when quantities exceed the body needs, it can become a pro-oxidant.

A complete multi-faceted approach to nutrition and disease
Many terms have come to define the complex symptom set and chronic disease inter-relationship that is generally recognized as the metabolic syndrome, a precursor to the modern scourge, adult- onset diabetes. Encompassing elevated lipids and glucose, high blood pressure, central adiposity and other signs and symptoms, this is the clinical manifestation of the American obesity epidemic. Nutrition texts refer to a toxic food environment.
Each quarter, I explain to students that this phrase has a dual meaning. We live in a predominantly couch-friendly, TV marketing environment that is toxic, with ads featuring highly refined foods. At the same time, we are eating food that is, in fact, toxic, adding to the metabolic burden on our bodies. Though modern medicine relies on various pharmacological therapies, reversing diabetes and obesity must focus foremost on education, prevention and lifestyle change. Diet and exercise ought to lay the groundwork with herbs, specific phytochemicals and nutritional supplements playing supporting roles.
Modern Nutrition There is no question that our diet has changed drastically over the past 75 years. Factory food production has been geared towards corporate profits, with food preservation and refinement taking the lead, while nutrients have been stripped from the finished product. In more recent decades, greater sums of flavorings, colorings, stabilizers and preservatives have become part of factory food recipes, not to mention pesticides, agricultural - chemical residues, and of late, modified genes (GMO). The government and academic institutions have been complicit as well.
Take for example the old Department of Agriculture food pyramid with grains as the base; a recipe for diabetes. The new pyramid, laid out differently, is little improved. I advise my students to put vegetables, legumes, other plant foods, wild and grass-fed animal proteins, and some fruit near or at the base, and move grains (way) up the pyramid, if they choose to use this illustration at all. An entertaining online pursuit is perusing mypyramid.gov and then mypyramid.org, the former being, of course, the official site. The latter is a parody brought to my attention by a former student.
A decade ago, I attended an organic farming class at Rutgers, the State University of New Jersey. It took place in the Center for Advanced Food Technology and Nabisco Advanced Food Technology Institute. Clearly, industry money is influencing research, especially in an environment of financially strapped state colleges. In fact, a recent study on nutrition research out of Boston showed that four out of five studies funded by food industries were biased to show results favorable to their sponsors. In food processing, one of the first macro-nutrient components to be removed was dietary fiber, the matrix that holds plants together and incorporates key phytochemicals in its structure.
Fiber There is a direct relationship between the inclusion of dietary fiber in meals and the bodys glycemic response to specific foods. Soluble, viscous fibers especially slow glucose absorption by slowing transit of food through the upper gastrointestinal tract. Found in legumes, citrus whites, apple skins, berries, various fruits, grains and other food, pectin, gum and mucilage can also lower blood cholesterol by binding with bile acids and aiding their excretion.
Much research is being conducted on how the body utilizes both soluble and insoluble fiber. Many think fiber goes unused. However, soluble fiber is partially broken down in the gut, yielding energy, vitamin manufacture and immune enhancement by nourishing flora and by extension, colonocytes. Insoluble fiber aids passage through the lower digestive tract and plays other roles. By eating more fibrous foods in general, we are likely to consume more health-promoting constituents in addition to phytochemicals. Lignans, for example, perform phenomenal tasks in the body though a lesser role in lowering risk of diabetes. For 75 cents per pound, one can purchase lignan-laden flaxseeds that may prevent and control breast, prostate and other cancers.
Several companies have combination formulas of powdered, soluble fiber-rich blends. These are often a considerable step in quality above commercial brands and are sometimes made without psyllium and oats, to which some individuals are intolerant/allergic. Encapsulated products are also available, though more costly, but may boost individuals adherence to a nutritional program. Powdered fiber products are invaluable for increasing overall fiber consumption and slowing glucose absorption. In fact, some individuals would get more fiber in 1 or 2 scoops of a particular fiber formula than they may get in an entire days diet. The FDAs daily value for fiber is set at 25 grams; however, few Americans consume this quantity. It is telling that traditional cultures, many of which ate 3-5 times as much fiber, had no diabetes. Compare these quantities to the average Americans daily intake of added sugar, approximately 30 teaspoons per day.
Sugar and Spice I often ask students to consider the historic availability of sugar (and most commonly used vegetable oils). I once heard the respected herbalist, David Winston, speak of the same. Obtaining sugar was a difficult, seasonal process at best. Sugar was available certain times of the year from whole foods, fruits and vegetables, not in soda, juice, and an array of cookies and candies. It was prized and stored in the whole food matrix, dried, used to ferment, etc. The takeaway: instruct your patients and clients to step out of the box and eat whole foods rich in fiber and phytochemicals. Furthermore, we can take advice from NYU nutritionist Marion Nestle and walk the perimeter of grocery stores. The healthier foods generally live along the walls, and there appears to be less shelf-talking advertising.
Our ancestors knew there was more to spice than flavor. Cinnamon, like flaxseed, is an inexpensive food that can have profound health effects. Studies show that a gram or two per day of cinnamon may lower serum glucose, triglycerides, LDL cholesterol and total cholesterol. More than 170 million people worldwide suffer from diabetes, and for many, drugs or other forms of treatment are unavailable. It may be possible that many of these people could benefit from readily available natural products such as cinnamon, according to Don Graves, Adjunct Professor of Molecular, Cellular, and Developmental Biology from the University of California at Santa Barbara. Using advanced imaging techniques, Graves and other researchers found a proanthocyanidin compound in cinnamon that can affect insulin signaling in fat cells, thus proposing that cinnamon has insulin-like activity. This may not be news to many practitioners, but it is likely to be surprising and interesting to the average patient who has no idea that a simple kitchen spice can be so powerful.
Preliminary studies from the Japanese National Institute of Health Sciences show that chocolate may slow or prevent pancreatic carcinogenesis. Chocolate also contains proanthocyanidins and has diverse health benefits. Raw, sugarless cacao powders are available on the market. These can be mixed with chai, which usually contains cinnamon (and other phytochemical-bearing herbs), or concentrated foods such as solid extracts of berries.
Common to the Southwest, the prickly pear (Opuntia spp.) may offer promise in diabetes treatment and management. Mexicans eat the pads of this cactus, a notable source of gut-soothing mucilage, as nopalitos. The plants range is actually quite far-reaching as I have seen it on barrier islands off the Jersey Shore. In Spanish, the fruit of prickly pear is called tuna. It is rich in quercetin, a neuro-protective flavonoid common to many vegetables, fruits and herbs. Prickly pear may at least provide a demonstrative link for those with Type 2 diabetes. Traditional and available ethnic foods, including okra - another mucilage-bearing food can be used in preventing diabetes because the mucilage slows the absorption of sugars. Meal planning may include soluble fiber rich Anasazi or black beans, lima beans, or black eyed peas, along with nopalitos or okra.
Distinct medicinal food-herbs are also valuable dietary options. Herbs high in mucilage are slippery elm (Ulmus rubra) bark and marshmallow (Althea officinalis) root. Made into an unboiled tea, an infusion, the herbs thicken into a goo that provides a slimy, protective layer to the lining of an irritated digestive tract or sore throat. Moreover, diabetics should eat sources of complex sugars such as inulin found in Jerusalem artichokes (Helianthus tuberosus) instead of potatoes. Complex sugars from Helianthus and other foods, such as asparagus and salsify (Tragopogon porrifolius) digest more slowly, thus less sugar builds up in the bloodstream. These foods also fortify the lower digestive tract by nourishing gut flora.
Nutrient and Phytochemical Supplements Prevention and management of diabetes can be enhanced with nutrients and phytochemical supplements. Alpha lipoic acid, for example, is a versatile nutrient for diabetic treatment. In addition to inhibiting glycation and therefore helping to prevent diabetic complications, such as diabetic neuropathy, lipoic acid performs other roles in antioxidant recycling and manufacture, and promotes efficient cellular energy production. For treatment purposes, 600-1,200 mg daily are recommended, while 100-300 mg daily may be recommended for prevention. Alpha lipoic acid is also helpful in treating chemotherapy-induced neuropathy and may offer hope to those with fluoroquinolone antibiotic-induced neuropathy as well.
Other nutrients critical to diabetes prevention and management are B vitamins, which are needed for metabolism of all macronutrients, and may also aid in treatment of neuropathy. Trace minerals such as vanadium and chromium play crucial roles, and few strategies for ongoing wellness are complete without additional magnesium and zinc, as they are vital to numerous enzymatic reactions and are frequently deficient in aging populations.
Generally, for those with insulin resistance, 10-40 mg of supplemental zinc and 100-300 mg magnesium are sufficient for enhancing immune response and reducing cardiovascular risk, respectively. Both minerals also contribute to bone health. Various studies show that chromium stabilizes blood sugar by enhancing insulins activity. In other studies, individuals who took additional chromium (200 mcg per day) gained muscle and lost fat in greater sums than controls. Improved body composition, body mass index and shrinkage in waist circumference (central adiposity) are directly linked to better outcomes for diabetes patients.
Many of the nutrients critical to blood sugar management are often combined in quality formulas available through healthcare practitioners. They now often contain vanadium, which decreases plasma glucose levels and has an insulin-like effect. Insulin and glucagon are not the only important hormones related to diabetes. According to Donald Yance, excess insulin in the bloodstream promotes the production of estrogen and reduces the level of sex hormone binding globulin (SHBG). SHBG is a plasma protein that binds and transports sex hormones, including estrogen and testosterone. SHBG receptor interaction depends on the occupancy of the steroid binding site. It is recommended to measure total testosterone, free testosterone, SHBG, DHEA sulfate and a thyroid panel.
Low testosterone with high estrogen can also cause an increase in fat storage. Studies have found that serum testosterone is decreased in markedly obese males. This is due to a reduction in SHBG. This can cause peripheral conversion of testosterone to estradiol, which can cause fat accumulation as well as lean muscle loss.
SHBG is also critically important to proper insulin control, as well as estrogen. A reduction in SHBG is associated with hyperinsulinism and insulin resistance. High fasting insulin levels are noted to be an independent predictor for ischemic heart disease in men. According to a recent study, low androgen levels in men correlated with increased risk of atherosclerosis. Yances Natura Health Products offers a supplement called Night Gain. It combines testosterone-boosting and estrogen-metabolizing nutrients and phytochemicals with amino acids such as arginine for cardiovascular benefits. Night Gain enhances sexual function and sleep and is helpful to both sexes, with or without insulin resistance.
Generally, I recommend 2-6 caps in divided doses in the evening. Milk thistle (Silybum marianum) offers promise in treating diabetes. In a recent study found in Phytotherapy Research, moderate dosing of roughly 600 mg silymarin daily has shown to lower various markers associated with diabetes including hemoglobin A1c (HbA1c). Using milk thistle is especially advantageous because of its protective effect upon the liver, an organ greatly stressed in obese individuals. It may also prevent prostate cancer and is currently being used for treating the disease.
Liquid herbal extracts are especially useful for diabetes management because of their relatively rapid digestion and absorption. A useful liquid formula for mild insulin resistance is Pancreaid, a product from Herbalist and Alchemist, formulated by herbalist David Winston. The blend contains dandelion root (Taraxacum off.), blueberry leaf (Vaccinium spp.), gentian root (Gentiana lutea), Devils Club root bark (Oplopanax horridus), Gymnema (Gymnema sylvestre) and cinnamon (Cinnamomum cassia). Bitters like dandelion and gentian roots have traditionally been used to normalize blood sugar levels and to help the beta cells in the pancreas to utilize endogenous insulin. Blueberry leaf, Devil’s Club and Gymnema have been used throughout the world for adult onset diabetes. Studies on Gymnema show a pronounced amphoteric effect on the Isles of Langerhans in the pancreas—nourishing, strengthening and normalizing function, according to Winstons manual, Herbal Therapeutics: Specific Indications for Herbs and Herbal Formulas.
Anthocyanins The leaves of blueberry are not the only useful part. The pigments that come from blueberries (bilberries), black currants and other berries, as well as the skins of some vegetables anthocyanins have marked therapeutic effects. I recommend a highly concentrated 80 mg anthocyanin capsule (MEDOX Optimal) for any client with diabetes to slow the multisystemic degeneration associated with the condition.
MEDOX is a veg-encapsulated anthocyanin extract from wild Norwegian bilberries (Vaccinium myrtillus) and black currants (Ribes nigrum). Anthocyanins from bilberry are perhaps best known for the roles they play in protecting the eyes from injury related to oxidative stress and poor capillary microcirculation, such as macular degeneration and cataracts. Bilberry anthocyanins improve night vision and retinopathies. Mitigating and reversing complications related to diabetes and insulin resistance is a unique domain of anthocyanins in that they protect the blood vessels, eyes and kidneys. Furthermore, studies from Ulleval University Hospital in Norway indicate that 2 caps daily of MEDOX Optimal lowered C-reactive protein (a marker of inflammation) by 28% after two weeks. Though this study was performed on a small sample size, compare the results to 17% reduction for statin drugs, which bear considerable side effects.

Each quarter, I explain to students that this phrase has a dual meaning. We live in a predominantly couch-friendly, TV marketing environment that is toxic, with ads featuring highly refined foods. At the same time, we are eating food that is, in fact, toxic, adding to the metabolic burden on our bodies. Though modern medicine relies on various pharmacological therapies, reversing diabetes and obesity must focus foremost on education, prevention and lifestyle change. Diet and exercise ought to lay the groundwork with herbs, specific phytochemicals and nutritional supplements playing supporting roles.
Modern Nutrition There is no question that our diet has changed drastically over the past 75 years. Factory food production has been geared towards corporate profits, with food preservation and refinement taking the lead, while nutrients have been stripped from the finished product. In more recent decades, greater sums of flavorings, colorings, stabilizers and preservatives have become part of factory food recipes, not to mention pesticides, agricultural - chemical residues, and of late, modified genes (GMO). The government and academic institutions have been complicit as well.
Take for example the old Department of Agriculture food pyramid with grains as the base; a recipe for diabetes. The new pyramid, laid out differently, is little improved. I advise my students to put vegetables, legumes, other plant foods, wild and grass-fed animal proteins, and some fruit near or at the base, and move grains (way) up the pyramid, if they choose to use this illustration at all. An entertaining online pursuit is perusing mypyramid.gov and then mypyramid.org, the former being, of course, the official site. The latter is a parody brought to my attention by a former student.
A decade ago, I attended an organic farming class at Rutgers, the State University of New Jersey. It took place in the Center for Advanced Food Technology and Nabisco Advanced Food Technology Institute. Clearly, industry money is influencing research, especially in an environment of financially strapped state colleges. In fact, a recent study on nutrition research out of Boston showed that four out of five studies funded by food industries were biased to show results favorable to their sponsors. In food processing, one of the first macro-nutrient components to be removed was dietary fiber, the matrix that holds plants together and incorporates key phytochemicals in its structure.
Fiber There is a direct relationship between the inclusion of dietary fiber in meals and the bodys glycemic response to specific foods. Soluble, viscous fibers especially slow glucose absorption by slowing transit of food through the upper gastrointestinal tract. Found in legumes, citrus whites, apple skins, berries, various fruits, grains and other food, pectin, gum and mucilage can also lower blood cholesterol by binding with bile acids and aiding their excretion.
Much research is being conducted on how the body utilizes both soluble and insoluble fiber. Many think fiber goes unused. However, soluble fiber is partially broken down in the gut, yielding energy, vitamin manufacture and immune enhancement by nourishing flora and by extension, colonocytes. Insoluble fiber aids passage through the lower digestive tract and plays other roles. By eating more fibrous foods in general, we are likely to consume more health-promoting constituents in addition to phytochemicals. Lignans, for example, perform phenomenal tasks in the body though a lesser role in lowering risk of diabetes. For 75 cents per pound, one can purchase lignan-laden flaxseeds that may prevent and control breast, prostate and other cancers.
Several companies have combination formulas of powdered, soluble fiber-rich blends. These are often a considerable step in quality above commercial brands and are sometimes made without psyllium and oats, to which some individuals are intolerant/allergic. Encapsulated products are also available, though more costly, but may boost individuals adherence to a nutritional program. Powdered fiber products are invaluable for increasing overall fiber consumption and slowing glucose absorption. In fact, some individuals would get more fiber in 1 or 2 scoops of a particular fiber formula than they may get in an entire days diet. The FDAs daily value for fiber is set at 25 grams; however, few Americans consume this quantity. It is telling that traditional cultures, many of which ate 3-5 times as much fiber, had no diabetes. Compare these quantities to the average Americans daily intake of added sugar, approximately 30 teaspoons per day.
Sugar and Spice I often ask students to consider the historic availability of sugar (and most commonly used vegetable oils). I once heard the respected herbalist, David Winston, speak of the same. Obtaining sugar was a difficult, seasonal process at best. Sugar was available certain times of the year from whole foods, fruits and vegetables, not in soda, juice, and an array of cookies and candies. It was prized and stored in the whole food matrix, dried, used to ferment, etc. The takeaway: instruct your patients and clients to step out of the box and eat whole foods rich in fiber and phytochemicals. Furthermore, we can take advice from NYU nutritionist Marion Nestle and walk the perimeter of grocery stores. The healthier foods generally live along the walls, and there appears to be less shelf-talking advertising.
Our ancestors knew there was more to spice than flavor. Cinnamon, like flaxseed, is an inexpensive food that can have profound health effects. Studies show that a gram or two per day of cinnamon may lower serum glucose, triglycerides, LDL cholesterol and total cholesterol. More than 170 million people worldwide suffer from diabetes, and for many, drugs or other forms of treatment are unavailable. It may be possible that many of these people could benefit from readily available natural products such as cinnamon, according to Don Graves, Adjunct Professor of Molecular, Cellular, and Developmental Biology from the University of California at Santa Barbara. Using advanced imaging techniques, Graves and other researchers found a proanthocyanidin compound in cinnamon that can affect insulin signaling in fat cells, thus proposing that cinnamon has insulin-like activity. This may not be news to many practitioners, but it is likely to be surprising and interesting to the average patient who has no idea that a simple kitchen spice can be so powerful.
Preliminary studies from the Japanese National Institute of Health Sciences show that chocolate may slow or prevent pancreatic carcinogenesis. Chocolate also contains proanthocyanidins and has diverse health benefits. Raw, sugarless cacao powders are available on the market. These can be mixed with chai, which usually contains cinnamon (and other phytochemical-bearing herbs), or concentrated foods such as solid extracts of berries.
Common to the Southwest, the prickly pear (Opuntia spp.) may offer promise in diabetes treatment and management. Mexicans eat the pads of this cactus, a notable source of gut-soothing mucilage, as nopalitos. The plants range is actually quite far-reaching as I have seen it on barrier islands off the Jersey Shore. In Spanish, the fruit of prickly pear is called tuna. It is rich in quercetin, a neuro-protective flavonoid common to many vegetables, fruits and herbs. Prickly pear may at least provide a demonstrative link for those with Type 2 diabetes. Traditional and available ethnic foods, including okra - another mucilage-bearing food can be used in preventing diabetes because the mucilage slows the absorption of sugars. Meal planning may include soluble fiber rich Anasazi or black beans, lima beans, or black eyed peas, along with nopalitos or okra.
Distinct medicinal food-herbs are also valuable dietary options. Herbs high in mucilage are slippery elm (Ulmus rubra) bark and marshmallow (Althea officinalis) root. Made into an unboiled tea, an infusion, the herbs thicken into a goo that provides a slimy, protective layer to the lining of an irritated digestive tract or sore throat. Moreover, diabetics should eat sources of complex sugars such as inulin found in Jerusalem artichokes (Helianthus tuberosus) instead of potatoes. Complex sugars from Helianthus and other foods, such as asparagus and salsify (Tragopogon porrifolius) digest more slowly, thus less sugar builds up in the bloodstream. These foods also fortify the lower digestive tract by nourishing gut flora.
Nutrient and Phytochemical Supplements Prevention and management of diabetes can be enhanced with nutrients and phytochemical supplements. Alpha lipoic acid, for example, is a versatile nutrient for diabetic treatment. In addition to inhibiting glycation and therefore helping to prevent diabetic complications, such as diabetic neuropathy, lipoic acid performs other roles in antioxidant recycling and manufacture, and promotes efficient cellular energy production. For treatment purposes, 600-1,200 mg daily are recommended, while 100-300 mg daily may be recommended for prevention. Alpha lipoic acid is also helpful in treating chemotherapy-induced neuropathy and may offer hope to those with fluoroquinolone antibiotic-induced neuropathy as well.
Other nutrients critical to diabetes prevention and management are B vitamins, which are needed for metabolism of all macronutrients, and may also aid in treatment of neuropathy. Trace minerals such as vanadium and chromium play crucial roles, and few strategies for ongoing wellness are complete without additional magnesium and zinc, as they are vital to numerous enzymatic reactions and are frequently deficient in aging populations.
Generally, for those with insulin resistance, 10-40 mg of supplemental zinc and 100-300 mg magnesium are sufficient for enhancing immune response and reducing cardiovascular risk, respectively. Both minerals also contribute to bone health. Various studies show that chromium stabilizes blood sugar by enhancing insulins activity. In other studies, individuals who took additional chromium (200 mcg per day) gained muscle and lost fat in greater sums than controls. Improved body composition, body mass index and shrinkage in waist circumference (central adiposity) are directly linked to better outcomes for diabetes patients.
Many of the nutrients critical to blood sugar management are often combined in quality formulas available through healthcare practitioners. They now often contain vanadium, which decreases plasma glucose levels and has an insulin-like effect. Insulin and glucagon are not the only important hormones related to diabetes. According to Donald Yance, excess insulin in the bloodstream promotes the production of estrogen and reduces the level of sex hormone binding globulin (SHBG). SHBG is a plasma protein that binds and transports sex hormones, including estrogen and testosterone. SHBG receptor interaction depends on the occupancy of the steroid binding site. It is recommended to measure total testosterone, free testosterone, SHBG, DHEA sulfate and a thyroid panel.
Low testosterone with high estrogen can also cause an increase in fat storage. Studies have found that serum testosterone is decreased in markedly obese males. This is due to a reduction in SHBG. This can cause peripheral conversion of testosterone to estradiol, which can cause fat accumulation as well as lean muscle loss.
SHBG is also critically important to proper insulin control, as well as estrogen. A reduction in SHBG is associated with hyperinsulinism and insulin resistance. High fasting insulin levels are noted to be an independent predictor for ischemic heart disease in men. According to a recent study, low androgen levels in men correlated with increased risk of atherosclerosis. Yances Natura Health Products offers a supplement called Night Gain. It combines testosterone-boosting and estrogen-metabolizing nutrients and phytochemicals with amino acids such as arginine for cardiovascular benefits. Night Gain enhances sexual function and sleep and is helpful to both sexes, with or without insulin resistance.
Generally, I recommend 2-6 caps in divided doses in the evening. Milk thistle (Silybum marianum) offers promise in treating diabetes. In a recent study found in Phytotherapy Research, moderate dosing of roughly 600 mg silymarin daily has shown to lower various markers associated with diabetes including hemoglobin A1c (HbA1c). Using milk thistle is especially advantageous because of its protective effect upon the liver, an organ greatly stressed in obese individuals. It may also prevent prostate cancer and is currently being used for treating the disease.
Liquid herbal extracts are especially useful for diabetes management because of their relatively rapid digestion and absorption. A useful liquid formula for mild insulin resistance is Pancreaid, a product from Herbalist and Alchemist, formulated by herbalist David Winston. The blend contains dandelion root (Taraxacum off.), blueberry leaf (Vaccinium spp.), gentian root (Gentiana lutea), Devils Club root bark (Oplopanax horridus), Gymnema (Gymnema sylvestre) and cinnamon (Cinnamomum cassia). Bitters like dandelion and gentian roots have traditionally been used to normalize blood sugar levels and to help the beta cells in the pancreas to utilize endogenous insulin. Anthocyanins The leaves of blueberry are not the only useful part. The pigments that come from blueberries (bilberries), black currants and other berries, as well as the skins of some vegetables anthocyanins have marked therapeutic effects. I recommend a highly concentrated 80 mg anthocyanin capsule (MEDOX Optimal) for any client with diabetes to slow the multisystemic degeneration associated with the condition.

Top 7 Tips To Treat And Prevent Tennis Elbow
You don’t have to play tennis to get tennis elbow. In fact, about 95 percent of folks with this condition never set foot on a court. Instead, they garden, they type, they turn wrenches, they carry briefcases, activities that require them to repeatedly rotate the elbow or flex the wrist, usually while gripping a heavy object. Like a good backhand, tennis elbow takes time to develop. The first sign is usually soreness or a dull ache on the outside of the elbow joint that gets worse when you grasp something. Eventually, the pain may radiate down the top of your forearm, sometimes all the way to your wrist. Here are some tips that you can consider to adopt to relieve your elbow pain.
1. Pick Up Where You Left Off
You can ease back into your normal routine when your elbow no longer bothers you. As a general rule, there should be no pain associated with day-to-day tasks before you move on to something more demanding. Give yourself time to see how your elbow reacts. Don’t overdo it just because you don’t feel pain right away.
2. Use Soothing Strokes
Relaxing the surrounding muscles can take some of the pressure off an aching elbow. Gently massage the full length of your forearm muscle from your elbow to above your wrist, not just where you feel pain.

Another feature that is fairly unique is the use of air resistance. On cheap machines the resistance is set by manually adjusting a belt, whereas on more expensive models you get magnetic resistance that can be altered at the push of a button. However, on this Schwinn exercise bike, the air resistance means that the faster you pedal the more resistance you get; the slower you pedal the less resistance.
This bike, although good, isn't going to appeal to everyone. For starters, there's no heart rate monitor - for many this is a vital feature of a bike. Also, the LCD display is rudimentary. The readouts are Time, Distance, Calories, Calories per Hour, RPM, and Workload. 2). "Don't let yourself get hungry. Don't arrive at the party starving. Be sure you eat your ordinary, disease-free meals, especially breakfast. A protein rich breakfast "resets" the body and starts it off not hungry. Don't starve yourself, thinking you can "save up" calories. Fill up before the big holiday feasts with invigorating vegetable snacks." [Standard? There ain't no stinking "usual" in November and December, and Jennifer's not sure she ever has usual, sustaining meals. She's got a life to run. Jennifer doesn't' know where YOU'RE having Thanksgiving, but Jennifer's family's appetizer table has God's own Cheese Puffs and Ruffles with sour cream/onion dip, Brie on sourdough and 80 plates of cookies and bowls of peanut M&Ms on every applicable surface! There is nary a vegetable snack in sight.]
3) "How can you keep the pounds off at calorie rich parties? Stay away from the food! Literally ... stay on the other side of the room from the buffet table or appetizers tray. [Jennifer liked this one. She'll just go plant herself in the bathroom and lock the door. Maybe she can acquire a place under the pile of coats in the back bedroom. A nap sounds delightful.]
4). "Wear clothes which are slightly tight and your favorites. You'll think twice about the third helping if you can't let your belt out a notch. Always remember there's about 8 pounds between dress sizes." [What skinny sadist came up with this bright idea? Jennifer bets she's a size 2, and hides an eating disorder!]
5). "Portion size is the real mystical. Keep salad portions excessive and all other portions extra miniscule. When it comes to sweets, think attribute, not availability. Just because the candy corn is there doesn't mean you have to eat it. Don't be afraid to cut off "just a bite" of a expensive calorie handle. Put the rest back on the serving tray. If you're the hostess, pre-cut excessive calorie items into tiny portions to help your friends. Just a bite may be plenty to satisfy you. [Just a bite! Just a bite? Jennifer's stressed to the max and darn it, Jennifer DESERVES a candy treat ... and a WHOLE one. Heck, Jennifer's double stressed, so she deserves ...]
6). "You don't need to be stressed. Take Time for You. Get a massage or a delightful facial. Sit down and slip your shoes off." [Don't you know Jennifer's facing a month and a half on shopping overdrive and the only place to sit down is at the mall's breakneck-food court? McDonald's doesn't have a massage spa. If Jennifer slips her shoes off, she'll NEVER get them back on again, you fool.]
7). "Keep up your food diary ... every day of the holiday ... it will assistance you pinpoint your certain weaknesses. Just one 150 calorie chocolate chip cookie each day will add up to an spare pound in only 3 weeks." [Jennifer can't even obtain her food diary in chaos house. And, she really needed the enlightenment on the cookies. Thank you very much. Jennifer just ate three, and she doesn't need any food diary to pinpoint this tiny weakness.]

Jennifer bets Lucille's glad she got granddad's wiry build instead of those astounding wide childbearing hips from grandma's "peasant" stock!]
9). Drink water instead of alcohol which has empty calories and lowers will power. Avoid sugary sodas which throw your metabolism into pendulum swings. [You didn't mention eggnog. Eggnog is OK, then, right?]
10). "Don't eat while you cook. Those petite "tastes" can turn into 1000 calories before you've blinked." [Jennifer just spent 304 hours in the kitchen, cooking. She HAS to taste everything before she serves it, or they'll hate it and won't delight in her any numerous. And besides, cookie dough doesn't have any calories, does it?]
